This website, daytonabeachdentalimplants.com, is operated by Bergens Periodontics & Implant Dentistry, 724 South Beach Street, Ste 4, Daytona Beach, FL 32114. This policy explains what information the website collects, how it is used, and the choices you have.

Information you give us

The only forms on this website are the appointment request form on the Request Appointment and Contact pages. When you send a request, we collect:

• Your name, phone number, and email address
• The office and time of day you prefer
• Anything you type in the message box

We use this information to contact you about scheduling and to answer your question. Along with the form, the website records the date and time, the internet (IP) address your request came from, your browser type, and a spam-check score from Google reCAPTCHA (described below).

Your request is stored in the website's administration system, which only the practice's website administrators can sign in to, and a copy is emailed to the practice's scheduling team. [REVIEW: confirm how long requests are kept and who at the practice receives the notification email.]

Please do not include detailed medical information in the message box. The form is for scheduling. If you have a clinical question, call the office.

Information collected automatically (cookies)

Cookies are small text files a website stores in your browser. This website sets the following cookies.

Cookies the website needs to work

• Session cookie (named after the practice, ending in -session): keeps your visit together as you move between pages and lets the appointment form submit correctly. Lasts up to two hours. Set by this website.
• XSRF-TOKEN: protects the appointment form against forged submissions from other sites. Lasts up to two hours. Set by this website.

Analytics cookies (Google)

• _ga and _ga_0MG9Y064Y8: Google Analytics 4. Distinguish one visitor from another so we can see how many people visit, which pages they read, and how they found the site. Last up to two years, as set by Google.
• _gid and _gat_UA-177219537-9: Google Analytics. Distinguish visitors and limit how often data is sent. Last up to 24 hours and one minute respectively, as set by Google.

These analytics tags are loaded through Google Tag Manager (container GTM-NH38LQ7F). Analytics data is pseudonymous visitor data (pages viewed, approximate location, device and browser type). We do not send the contents of the appointment form to Google Analytics. [REVIEW: the Google Tag Manager container is managed outside this website. Confirm with the marketing team that it loads only the tags listed here.]

Spam protection (Google reCAPTCHA)

The appointment form uses Google reCAPTCHA v3 to tell people from automated bots. reCAPTCHA collects hardware and software information such as device and application data and sends it to Google for analysis, and may set a cookie on a google.com domain. Use of reCAPTCHA is subject to the Google Privacy Policy and Terms of Service. reCAPTCHA does not receive what you type into the form.

Video (YouTube)

Some pages include a video that is loaded from YouTube in privacy-enhanced mode only after you press play. YouTube may then set its own cookies under the Google Privacy Policy. Nothing is loaded from YouTube until you choose to play the video.

Fonts

Web fonts are served by bunny.net, which does not set cookies.

Who else receives information

• Google LLC: analytics (Google Analytics, Google Tag Manager), spam protection (reCAPTCHA), and video (YouTube), as described above.
• Twilio SendGrid: delivers the email notification of your appointment request to the practice.
• Placement Labs: builds and maintains this website on the practice's behalf and can access the website's administration system.
• Hosting provider: the website runs on a cloud server managed through Laravel Forge. [REVIEW: confirm the hosting entity the practice wishes to name, if any.]

We do not sell your information, and we do not use advertising or social media tracking pixels on this website.

Your choices

• Browser settings. Every major browser lets you block or delete cookies. Blocking the two cookies the website needs to work will stop the appointment form from submitting; you can still call either office.
• Google Analytics. You can opt out of Google Analytics across all websites with the Google Analytics Opt-out Browser Add-on.
• Do Not Track. This website does not currently respond to browser "Do Not Track" signals.

Security

Pages and forms on this website are served over an encrypted (HTTPS) connection. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

Children
