Completing your paperwork before your appointment means less time at the front desk and more time with Dr. Bergens. When you request an appointment, our scheduling coordinator will confirm your visit and make sure you have the registration forms you need.

What to bring to your first visit
- Your completed registration paperwork
- A photo ID and your insurance card, if you have coverage
- A list of any medications you are currently taking
- Any referral paperwork or X-rays from your general dentist
If anything on the form is unclear, call the office and a member of our team will walk you through it. You are also welcome to arrive a few minutes early and complete the paperwork with us.
